Participation Requirements

The Ohio Ophthalmological Society (OOS) has a grant through the Ohio Department of Health to promote eye safety and to encourage students to use protective eyewear outside of the classroom. The Ohio ACTE has partnered with the OOS to support this initiative in the vocational school setting. If your school is selected, this form will be used as your commitment to fulfill all participation requirements. In order to be considered as a participant in the Protective Eyewear Program, you must agree to the following requirements:

Choosing eyewear for your classroom is easy! This year, we have partnered with W.W. Grainger, Inc. to help us provide the best protective equipment available to the most students possible in Ohio. Please visit the link below to view the eyewear being offered through our program and make your selection. The final decision regarding participation will be based on creating a diverse group representative of Ohio’s CTE population. If your school is selected this form will be used as your commitment to fulfill all participation requirements.
